This form must be completed and received by the Office of Academic Records at least three days prior to commencement. Unless otherwise specified on this form, your name will be read as it appears on your graduation application (the name that will appear on your diploma). If you want to double check what you submitted, go to your Student Self-Service dashboard in HuskyNet and click View Graduation Application.
