Academic Records Forms
The following forms are for the convenience of the students in regards to the Office of Academic Records:
- Request for a replacement diploma
- Enrollment Certification Request Form
- Add/Drop Form (Use this form for add/drop, closed courses, pass/fail designation & audit designation.)
- Request for Off Campus Transfer Credit (Currently enrolled or continuing students use this form for prior approval of transfer credit.)
- Readmission Application Form
- Request for Undergraduate/Graduate Course Notation (Transfer students use this form to request designated HCU course credit if general elective credit originally given.)
- Student Request for Official Withdrawal
- Visiting Student Application
- Request for VA Certification or Change in Schedule
- FERPA Information
- FERPA Authorization Form (Goes to FERPA form in Huskynet)
