Academic Records Forms
The following forms are for the convenience of the students in regards to the Office of Academic Records:
- Add/Drop Form (Use this form for add/drop, closed courses, pass/fail designation & audit designation)
- Change of Student Information (Update name, address, phone, or SSN)
- CLEP Exam Approval Form
- Course Notation Form (Transfer students use this form to request designated HCU course credit if general elective credit originally given)
- Enrollment Certification Request Form
- FERPA Authorization Form (Goes to FERPA form in HuskyNet)

- Replacement Diploma Request
- Request for Off Campus Transfer Credit (Currently enrolled or continuing students use this form for prior approval of transfer credit)
