The following forms are for the convenience of the students in regards to the Office of Academic Records:

• Add/Drop Form (Use this form for add/drop, closed courses, pass/fail designation & audit designation)
• Change of Student Information (Update name, address, phone, or SSN)
• CLEP Exam Approval Form
• Course Notation Form (Transfer students use this form to request designated HCU course credit if general elective credit originally given)
• Enrollment Certification Request Form
• FERPA Authorization Form (Goes to FERPA form in HuskyNet)
• FERPA Information
• Readmission Application Form
• Replacement Diploma Request
• Request for Off Campus Transfer Credit (Currently enrolled or continuing students use this form for prior approval of transfer credit)
• Student Request for Official Withdrawal
